We confirm your appointment date, time, and which physician you will see. New patient paperwork is available online in advance through the patient portal.

Your first appointment

A comprehensive neurological evaluation — typically 45–60 minutes. Bring your records, medication list, and insurance card.

  • Prior EEG reports or imaging (MRI, CT scans)
  • Video of a seizure episode if available — very helpful for diagnosis
  • Notes on seizure frequency, duration, and triggers
  • Prior neurology notes or hospital discharge summaries

For Children

  • Child's immunization records
  • School or developmental evaluations if available
  • Parent / guardian with legal authority to consent

Do I need a referral to see a neurologist?
